Can you take Benadryl with Zoloft?

Benadryl® (diphenhydramine) and Zoloft® (sertraline) may sometimes be taken together, but this combination requires careful prescriber-supervised consideration due to two distinct concerns: additive anticholinergic/sedating effects, and a potential mild serotonin-related interaction. Key Clinical Considerations

  • Diphenhydramine has anticholinergic properties including dry mouth, urinary retention, constipation, and cognitive effects. Sertraline also has mild anticholinergic properties. These effects may be additive.
  • Both substances may contribute to central nervous system sedation which may lead to increased sleepiness and a slower reaction time. Patients should not drive or operate heavy machinery when using both.
  • There is a possible serotonin-related concern when diphenhydramine is used with sertraline. The significance may vary based on dose, frequency of use, patient history, and other concurrent medications.
  • Long-term or frequent use of diphenhydramine (Benadryl®) in patients on antidepressants is not recommended without prescriber oversight.

⚠ Prescriber note: Patients should inform their prescriber if they regularly use Benadryl® alongside Zoloft® or any SSRI/SNRI.

Side Effects to Discuss with Your Prescriber

  • Excessive drowsiness or sedation
  • Dry mouth, blurred vision, anticholinergic effects such as constipation
  • Confusion or cognitive slowing, especially in older patients
  • Urinary difficulty

When to Seek Urgent Care

  • Agitation, rapid heart rate, high fever, muscle twitching — possible serotonin syndrome, seek emergency care immediately.
  • Severe confusion or inability to wake
