Can you take combined HRT with antidepressants?

Combined HRT (estrogen + progestogen) and antidepressants may commonly be used together.. This page is educational only. Therapy decisions should be determined by a licensed prescriber.

How They Can Safely Be Taken Together

Estrogen and progestogen work through hormone receptors, while antidepressants act through serotonin, norepinephrine, and dopamine pathways. Because they operate on different biological systems, they may not affect one another.

Most antidepressants are processed through liver enzymes and may not be altered by standard doses of estrogen or progesterone. Estrogen may support serotonin function, which may be complementary when combined with antidepressant therapy under prescriber supervision.

When Prescribers May Monitor More Closely

  • Antidepressants that strongly affect liver enzymes (such as paroxetine or fluoxetine): prescribers may prefer body-identical progesterone for more predictable metabolism.
  • When starting or stopping either medication: check-ins allow prescribers to ensure doses remain appropriate.
