Treatment plans will depend on blood pressure control, medical history, hormone type, dose, and route of administration. Oral and transdermal hormone therapy are not interchangeable from a risk standpoint. Combined HRT works through hormone receptors, while blood pressure medications act on blood vessel tone, kidney sodium handling, or heart workload. Because these are different pathways, these medications may be used together in some patients under prescriber supervision.
Modern transdermal and topical estrogens (patches, gels, sprays) are particularly gentle on blood pressure because they avoid first-pass liver metabolism. Most patients on blood pressure medications remain very stable when using these forms, as determined by their prescriber.
Any risks of interactions may be higher if:

There are no documented major drug-drug interactions between HRT and most antihypertensives. From a pharmacy interaction standpoint, they may be used under prescriber supervision in most patients.
Does combined HRT interact with amlodipine?
Combined HRT and amlodipine may be considered safe to use together when monitored by a prescriber. Mild fluid retention from HRT may occasionally affect blood pressure readings, which is why prescribers may recommend home monitoring during the first weeks.
Does combined HRT raise blood pressure while on medication?
For most patients, blood pressure remains stable on combined HRT. Oral estrogen may slightly raise BP in some patients. Transdermal and topical estrogens (patch, gel, spray) are gentler and may not affect BP. A prescriber may adjust HRT form or BP medication as needed.
Does combined HRT affect how ACE inhibitors work?
In most patients, combined HRT may not affect how ACE inhibitors lower blood pressure. Prescribers may monitor BP during the first weeks after starting or adjusting HRT.
Does combined HRT change beta-blocker effectiveness?
In most patients, combined HRT may not change how beta-blockers work. Prescribers should be notified of unusual dizziness, fainting, or a significantly slowed pulse.
