According to FDA-approved prescribing information for Prometrium, if you remember the same day, take the missed dose as soon as possible; if you do not remember until the next day, take your usual dose for that day and do not double up. Contact your prescriber if doses are missed repeatedly or if symptoms change.
Prometrium works by maintaining progesterone levels. A one-off missed capsule may have limited impact because progesterone's effects persist for many hours; however, missing multiple doses or several days in a row may lower progesterone enough to cause breakthrough bleeding, withdrawal symptoms, or reduced protection of the uterine lining. If you are on Prometrium as part of combined hormone therapy to prevent endometrial hyperplasia, consistency is important for endometrial protection. If you are using it for fertility or luteal-phase support, even a single missed dose may be significant—contact your reproductive specialist promptly.
